We hope that your visit to our office will be the best dental experience you ever have. That starts by ensuring you are well informed and not surprised. To ensure you are prepared for your first appointment please complete the following things:

  • Please complete the following forms and bring them with you to your first appointment
  • Read initial visit and regular checkups and office policies.
  • Send your dental records. Previous records can be helpful, and if you have had any current x-rays we may not have to take new ones.
  • Plan to ask questions. Our job is to teach you about your dental health and then to listen to how far you want to take that information. The more openly we communicate, the more confident and relaxed you will be.
  • Expect us to be on time. We make every effort to remain on schedule and we ask you to do the same. Should you need to reschedule your appointment, please call our office (636-391-6030) as soon as you realize there is a conflict which you cannot resolve.

We gladly devote the time and attention necessary in offering you financial counseling when discussing fees and methods of payment, because you deserve it!

To help keep costs down for you, our goal is to eliminate sending you a bill for any treatment received in our office. We can, therefore, avoid charging you more for procedures simply to cover the additional expenses in processing thousands of statements each year.

We ask that you pay us for your treatment by choosing one of our convenient payment options. We are proud to be able to help you save money whenever you visit us. By eliminating the costs associated with billing, we can pass the savings along to you!

Please take advantage of our courtesy adjustment whenever it's comfortable for you. Most of our patients love to save money! If you want to make payments, you can. Simply use a payment plan we've specially arranged to help you.

We believe it is important to provide both the highest quality dental care and to make this care affordable for our patients. We have made arrangements for our patients which allow payment to be convenient and flexible. We are committed to helping you receive the dental care you desire and the most pleasant dental experience possible.

We're Pleased to Offer You the Following Options for Your Convenience:

  • Payment in full at visit with cash, check, or credit card.

We offer a courtesy adjustment fee of 5% for advance payment of entire treatment plan if amount is greater than $300. We're proud to offer you the opportunity to save money on your dental treatment.

  • No-or low interest payment plans with no down payment necessary in most cases. At no charge, we will gladly process any insurance claim for you.
  • Estimated co-payment at visit with cash, check, or credit card with most insurance plans following benefit verification.

Please be aware that we cannot guarantee this estimate and that there may be a balance after insurance pays.

Note to Our Patients with Insurance. All our payment options help you to use your insurance in our office!

We are happy to process any insurance claim as a service to you at no charge. Please keep in mind that any estimate that we provide to you is only an estimate and that you are responsible for all fees in their entirety.

We are proud that our fees reflect the time that the doctor spends with each patient as well as the overall quality of care and service that we provide in our practice. Our fees are not based upon any insurance schedules, and are often above insurance allowances. We are happy to discuss your particular dental benefits with you.

  • Dental Insurance companies do not normally cover 100% of any dental fee and, and in many cases, cover less than 50% or sometimes make no payment. The "small print" written in the plan reveals the truth about actual percentages, which can be less than expected.
  • Your insurance plan is based upon a contract between your employer and the insurance company. Any dollar amount a plan reimburses for dental service is determined by how much your employer has paid for the plan. We will complete and submit all dental insurance forms to achieve the maximum coverage to which you are entitled.
  • Dental Insurance is not designed to pay your complete dental care. Many routine and newer services are often not covered. You are fortunate to have dental insurance that may assist you with the cost treatment. Our goal is to help you achieve and maintain optimal dental care. If you have any questions regarding the specifics of your dental insurance plan, please contact your employer or insurance company directly.
  • When dental insurance was introduced about fifty years ago, most annual maximums were $1,000. Most annual maximums remain today at approximately $1,000. Adjusted for inflation, your annual maximum should be over $5,000 today! While insurance premiums have increased, benefits have decreased.
  • Please be aware that certain carriers may not allow you to assign your benefits to our office. Because of this insurance restriction, we ask that you choose an alternative payment option. We gladly process your insurance claim for your direct reimbursement.

Emergency Policy

Registered patients in need of emergency dental care during non-office hours should call our office. Our phone system will prompt you to be able to leave a voicemail directly to the doctor's cell phone. We will return the call as soon as possible. Non-registered patients who have a true dental emergency can also be seen after hours. Our after hours fee is $165 and any treatment is to be paid in full at the time of the after hours visit. We will file your insurance for you to be directly reimbursed. In addition, registered patients who require an after hours visit to address treatment previously recommended but neglected will be subject to our after hour fees.

Cancellation policy

Our entire team is dedicated to providing every dental experience with excellent, personalized care and service to make your visits as comfortable and pleasant as possible. To make this a win-win relationship, we ask one thing of our patients, to respect the time that is reserved exclusively for you. We have established a fairly lenient cancellation policy to ensure that our patients respect the needs of other patients as well as the doctor's time.

Cancellations with more than 72 hours advance notice are not tracked.

Cancellations with less than 72 hours advance notice (short notice) are tracked for frequency. We reserve the right to charge $45 per hour for repeated no shows and cancellations without 48 hours notification.

Patients with repeated short notice cancellations are added to our short notice call list. From the list, our office calls patients to fill last minute openings. In addition, our short notice patients can call our office when they identify a free time in the day that works for them and we will be more than happy to see them if that time is available. This works well for patients who cannot predict their schedule in advance. Some patients opt to be added to the short notice list instead of rescheduling their appointments.

In cases where canceling and/or missed appointments become common occurrences and the above measures are ineffective our office requires a $200 deposit via credit card to reserve an appointment. We will then charge $45 per hour for no shows or cancellations without 48 hours advance notification.
